    Chrysalis Health, Inc., a leading community based behavioral healthcare agency is looking to fill a position for a Care Coordinator/Case Manager within our therapeutic group home for youth. This candidate will work directly with youth residing in the home by linking them to community resources, creating/following a service plan. This is a field based position which requires the candidate to both be a "road warrior" and meet with youth in two residential settings. The candidate will be responsible for collaborating with treatment teams, community agencies and advocating for the youth.




    •  Linking and facilitating with appropriate services and resources identified in the service plan.
    •  Represent and accompany the youth to court appointments and report the youth’s current progress and status.
    •  Advocating for the acquisition of services and resources necessary to implement the service plan.
    •  Documenting targeted case management activities and other clinical paperwork within the specified timeframe.
    •  Arranging for and coordinating after care services upon discharge from a residential or inpatient facility when discharge planning is not covered by the facility’s per diem.
    •  Demonstrates knowledge of the Medicaid manual and all updated revisions.
    •  Serving as a liaison for the client, family, referral source and other social service agencies.
    •  Ability to report to work as scheduled, in a timely fashion and will an attitude oriented toward service.
    •  Ability to understand and comply with all Avidity policies and procedures.



    • Must have Bachelor's degree in social work, psychology, behavioral health, or the like as per Medicaid guidelines
    • Must have 1 year working with populations served, as per Medicaid guidelines
    • Must successfully complete all criminal background screens annually
    • Must be able to successfully implement individualized treatment plans
    • Must have a valid Florida Driver’s License with a good driving record
